My Support My Way is hiring a Full-time employee for the position of Disabilities Services Officer. We are seeking an employee who is passionate about assisting and supporting people with a disability in all aspects of daily living.

Task & Responsibilities
Key Responsibilities:
- Assessing clients’ needs and planning, developing, and implementing educational, training and support programs.
- Interviewing clients and assessing the nature and extent of difficulties they are facing.
- Engaging directly with clients and their representatives to confirm their housing need and creating Support/Care plans based on functional capacity, client needs, and goals of the client.
- Referring clients to agencies that can provide additional support required.
- Build and maintain relationships with key external stakeholders, such as support
- coordinators, Behaviour Practitioners, plan managers, local area coordinators and others, with a goal to build strong connections and grow referrals.
- Building relationships with potential participants to establish rapport.
- Ensuring high levels of client satisfaction. Ensuring client and family members
- confidentiality is maintained.
- Providing and engaging Support Workers in regular training.
- Access Incident reports, Debrief Support workers and Clients and recommend necessary actions/measures to be taken to avoid future incidents.
- Send all the incident reports and Behaviour/ABC log data to the relevant stakeholders.
- To do regular visits to respite/SIL homes for internal Audit purpose.
- Conduct regular Support Staff and management meetings to discuss recommended/implemented strategies to managed behaviour of concerns and other support needs of the Client.
- Conduct meetings for policy reviews, implementing best practice guidelines and
- continuous improvement activities as required.
- Responsible as; Authorized Reporting Office, Registrant, Authorised Reportable
- Incidents Approver, Authorised Reportable Incidents Notifier, Worker Screening officer for My Support My Way at NDIS Quality and Safeguards Commission Portal.
- Responsible as: Authorised Program Officer for My Support My Way at DHS Restrictive Practice Unit.
- Doing weekly/monthly reporting to NDIS Quality and Safeguards Commission Portal and DHS Restrictive Practice Unit.
Qualification & Experience
Relevant qualification in disability services, at least at Diploma level.
At last 3 years of experience in a relevant field. Experience and knowledge of disability, mental health and/or positive behaviour support
Experience working in the area of complex behaviour support with people who need a stable and committed team around them
Committed to following Positive Behaviour Support Plans and able to demonstrate your experience working with behaviours of concern
Willing to learn new skills and adapt your approach to meet the individual needs of the person you support
Have a good understanding of person-centred practice
Have working entitlements to work within Australia
Must be flexible to work over a 7-day, 24-hour roster period
