Responsibilities:

Client Support and Care:

  • Provide direct support to clients with disabilities in accordance with their individualised support plans.
  • Assist with personal care tasks such as bathing, dressing, grooming, and toileting.
  • Administer medications as per prescribed guidelines and document accordingly.
  • Assist with mobility, transfers, and use of mobility aids.

Support Plan Implementation:

  • Implement individualised support plans developed in collaboration with clients, families, and other stakeholders.
  • Encourage clients to make choices and decisions regarding their daily activities and routines.
  • Monitor and evaluate client progress, making adjustments to support plans as necessary.

Emotional and Social Support:

  • Provide emotional support and companionship to clients, fostering a positive and caring environment.
  • Assist clients in developing social skills and maintaining relationships with family and friends.
  • Encourage participation in social and recreational activities within the community.

Behaviour Management and Crisis Intervention:

  • Use positive behaviour support techniques to manage challenging behaviours effectively.
  • Implement crisis intervention strategies as needed to ensure the safety and well-being of clients and others.

Documentation and Reporting:

  • Maintain accurate and up-to-date documentation of client interactions, progress notes, and incident reports.
  • Complete daily logs and reports as required by organisational policies and regulatory requirements.
  • Communicate effectively with supervisors and team members regarding client status, concerns, and achievements.

Health and Safety:

  • Ensure a safe environment for clients by following health and safety protocols and infection control measures.
  • Recognise and respond promptly to medical emergencies and provide basic first aid when necessary.

Professional Development:

  • Participate in ongoing training and development opportunities to enhance knowledge and skills in disability support.
  • Stay informed about best practices, industry standards, and regulatory changes affecting disability services.

Qualifications:

  • Previous experience in disability support, aged care, healthcare, or a related field preferred.
  • Certificate III in Individual Support (Disability) or equivalent qualification preferred.
  • First Aid and CPR certification (or willingness to obtain).
  • Strong interpersonal skills and the ability to communicate effectively with clients, families, and colleagues.
  • Ability to work independently and as part of a team in a dynamic and demanding environment.
  • Compassionate, patient, and respectful attitude towards individuals with disabilities.

Working Conditions:

  • Work may be performed in residential facilities, community settings, or clients’ homes.
  • Shift work may be required, including evenings, weekends, and public holidays.
  • Physical requirements include assisting with lifting, transferring, and personal care tasks.
