Disability Support Worker

About the role

Wellness Forever Support Services is seeking a Disability Support Worker to join our team in providing exceptional care and support to individuals with disability in our Supported Independent Living (SIL) program. As a casual Disability Support Worker, you will play a vital role in empowering our clients to live independently and achieve their goals, while ensuring their safety, health and wellbeing. This is an exciting opportunity to make a meaningful difference in the lives of those we support.

What you’ll be doing

• Providing personalised support and assistance with daily living activities, including personal care, meal preparation, household chores, and community engagement
• Implementing individual support plans to help clients achieve their independence and life goals
• Advocating for clients and collaborating with their support networks to ensure their needs are met
• Maintaining accurate records and documentation to monitor client progress and support
• Promoting a safe and inclusive environment that respects the rights, choices and dignity of our clients
• Participating in team meetings, training, and professional development opportunities to continuously improve your skills and knowledge

What we’re looking for

• A Certificate III or IV in Disability or equivalent qualification
• Experience working with individuals with disability in a support worker or care role
• Strong communication and interpersonal skills, with the ability to build rapport and provide empathetic support
• Excellent problem-solving and decision-making skills, with a commitment to person-centred care
• Flexibility and adaptability to work a variety of shifts, including weekends and public holidays
• A valid driver’s licence and access to reliable transportation
• A current First Aid Certificate and NDIS Worker Screening Check
