About you:
Join us as a Home Care Support Worker, where you’ll make a real difference in older clients’ lives by providing exceptional care to help them live safely and independently at home. You’ll play a key role in enhancing their wellbeing, ensuring safety, and fostering connections in line with their personalised Care Plan. You’ll document your care and promptly escalate any concerns, all while delivering a great client experience. If you’re compassionate and committed, we’d love to have you on our team!
Key Responsibilities
- Provide personalised, goal-oriented care for independent living.
- Promptly escalate client issues to a Coordinator or On Call.
- Communicate openly with clients, families, and advocates.
- Actively engage in individual and group care activities.
- Effectively manage time and inform clients of scheduling constraints.
- Guide clients to their Coordinator for service changes.
- Tailor support for culturally diverse clients.
- Assist clients in communicating service changes.
- Monitor and report changes in clients’ well-being.
Skills & Experience:
- Previous experience in community home care with older people.
- Proven ability to communicate effectively with clients and stakeholders.
- Experience in catering to culturally diverse older people – a plus.
- Ability to work autonomously with minimal supervision.
- Proficiency in understanding and following client Care Plans.
- Track record of building rapport with clients and stakeholders.
- Essential minimum qualification: Certificate III in Individual Support or Community Services.
