About you:

Join us as a Home Care Support Worker, where you’ll make a real difference in older clients’ lives by providing exceptional care to help them live safely and independently at home. You’ll play a key role in enhancing their wellbeing, ensuring safety, and fostering connections in line with their personalised Care Plan. You’ll document your care and promptly escalate any concerns, all while delivering a great client experience. If you’re compassionate and committed, we’d love to have you on our team!

Key Responsibilities

• Provide personalised, goal-oriented care for independent living.
• Promptly escalate client issues to a Coordinator or On Call.
• Communicate openly with clients, families, and advocates.
• Actively engage in individual and group care activities.
• Effectively manage time and inform clients of scheduling constraints.
• Guide clients to their Coordinator for service changes.
• Tailor support for culturally diverse clients.
• Assist clients in communicating service changes.
• Monitor and report changes in clients’ well-being.

Skills & Experience:

• Previous experience in community home care with older people.
• Proven ability to communicate effectively with clients and stakeholders.
• Experience in catering to culturally diverse older people – a plus.
• Ability to work autonomously with minimal supervision.
• Proficiency in understanding and following client Care Plans.
• Track record of building rapport with clients and stakeholders.
• Essential minimum qualification: Certificate III in Individual Support or Community Services.
